Abstract
Injury to the spleen or liver occurs in approximately one-third of patients following significant blunt abdominal trauma, therefore representing a common scenario for pediatric surgeons who manage trauma patients. The principles of management of blunt trauma to both organs are essentially the same. Canadian pediatric surgeons initiated and led the paradigm shift to splenic preservation following blunt abdominal trauma, starting with the pioneering publication by Upadhyaya and Simpson from the Hospital for Sick Children in 1968. Children are significantly more likely to undergo successful nonoperative management when treated by pediatric surgeons at pediatric trauma centers. The ATOMAC pediatric trauma consortium has supported a hemodynamically driven approach to management. Ongoing hemodynamic instability and bleeding requiring transfusion of more than 40 cc/kg packed red blood cells are also indicative of failed nonoperative management. The patient presented with significant hypotension with ongoing requirement for transfusion.
