Myeloproliferative disorders (MPD) are heterogeneous conditions associated with a proliferation of blood counts. Polycythemia rubra vera (PV), essential thrombocytosis/thrombocythemia (EV) and primary or idiopathic myelofibrosis are clonal disorders with similar genetic and clinical features. Major neurocognitive disorders, otherwise referred to as dementias are common in advancing age groups. Patients with neurocognitive disorders have heterogenous profiles. Reviewing medical histories and laboratory investigations and the exclusion of potentially reversible metabolic, electrolyte and other medical conditions are imperative in diagnosing and managing neurocognitive disorders. We report on 11 patients seen in the outpatient clinic setting for cognitive evaluation against the backdrop of suspected dementias. None of these patients had been diagnosed with a myeloproliferative disorder or other major hematological condition prior to the assessment. Patients with neurocognitive disorders have heterogeneous profiles. Reviewing medical histories and laboratory investigations are perquisites to making dementia diagnosis. The baseline characteristics of the patients revealed male predominance (n = 9; 81.82%), College or university education (n=5; 45.45%) JAK 2 v617 mutation analysis was undertaken. This was present in 7 patients. Clinical features of vascular and Alzheimer's dementia were identified in all the patients. One of the patients had persistent perceptual disturbances which led to a suspicion that he had Lewy body dementia. The prominent cognitive domains were with working memory (100%), visuospatial deficits (81.82%) and executive function (100%). Other cognitive deficits reported were inattention, disorientation and language (anomia and poor semantic and verbal fluencies). Functional and cognitive improvements following the diagnosis and management of these myeloproliferative conditions. Hydroxyurea (HU) was initiated in 8 patients with essential thrombocytosis and polycythemia rubra vera. Subjective and objective improvements in cognition and behaviors occurred in 4 patients who commenced HU. Three patients did not initiate HU therapy and 1 patient discontinued HU after 3 months of therapy. Our observation highlights the importance of identifying potentially reversible etiologies for cognitive impairment. Terms: Neurocognitive disorders, dementia(s), myeloproliferative disorders.
