Abstract Carcinoma of the stomach has undergone a number of modifications in the 7th TNM edition. T1 was subdivided to separate mucosal and submucosal invasion. The former T2a and b are now T2 (muscularis propria) and T3 (subserosa). The former T3 and T4 were changed to T4a (penetrates serosa) and T4b (invades adjacent structures). The division of regional lymph node involvement conforms to those of the oesophagus so that the gastric T, N, and M categories are now identical with those of the oesophagus (and gastroesophageal junction). The staging of carcinomas of the oesophagogastric/gastroesophageal junction has been addressed and the definition is noted in the oesophagus and stomach chapters.
