A psicopatologia operacional e seu uso como instrumento diagnóstico, promovido pelas versões atuais da Classificação Internacional de Doenças (CID) e o Diagnostic and Statistical Manual of Mental Disorders (DSM) permitiu a unificação da linguagem psiquiátrica e obteve algum sucesso na tentativa de aumentar a confiabilidade diagnóstica entre psiquiatras. A sua hegemonia, entretanto, debilitou a capacidade de realização do exame psíquico por psicopatólogos e reduziu o ensino de outros modelos psicopatológicos, como o fenomenológico, aos profissionais em formação. O presente estudo visa demonstrar a contribuição da fenomenologia ao exame psíquico e ao refinamento da capacidade diagnóstica. Neste trabalho, um caso clínico foi avaliado, simultaneamente, pelos modelos operacional e fenomenológico, demonstrando-se o processo de raciocínio diagnóstico envolvido em ambos, chegando-se ao diagnóstico de Transtorno de Ansiedade Generalizada (TAG), pela metodologia operacional, e de Esquizofrenia, pelo método fenomenológico. Assim, por este método, obteve-se o diagnóstico de uma condição psicopatológica mais grave e que requer medidas terapêuticas distintas daquelas exigidas para o TAG.
