目的 探讨初次人工全髋关节置换术(THA)后早期股骨假体周围骨折的治疗方法及疗效.方法 2006年5月至2010年9月共收治15例初次THA后早期股骨假体周围骨折患者,男6例,女9例;年龄为63 ~ 82岁,平均72.3岁.骨折距THA时间平均为62.5 d(3~87 d).骨折类型按Van-couver分型:AG型2例,AL型2例;B1型5例,B2型3例,B3型1例;C型2例.非手术治疗2例,手术治疗13例:切开钢丝环扎内固定2例,记忆合金环抱器固定3例,长柄生物型假体翻修术4例,锁定加压钢板内固定4例.记录患者的骨折愈合时间,比较患者术前与术后1年的疼痛视觉模拟评分(VAS)和髋关节Harris评分. 结果 15例患者术后获平均3.5年(1.5 ~5.0年)随访.12例患者于术后平均4.5个月(3.5 ~9.0个月)获骨性愈合,愈合率为80%.术后1年15例患者的疼痛VAS评分由术前平均(7.4+1.2)分降低至(2.3+1.1)分,髋关节Harris评分由术前平均(35.1+5.4)分提升至(75.4+7.2)分,术前与术后1年比较差异均有统计学意义(P<0.05).3例患者出现骨折延迟愈合.3例患者发生骨折不愈合,其中1例合并感染,均采取二期手术治疗.结论 对于初次THA后早期股骨假体周围骨折,根据不同Vancouver分型选择记忆合金环抱器、锁定钢板或假体翻修术治疗可取得满意疗效。
