筋萎縮性側索硬化症(ALS)は,病態解明が進み,治験薬の開発が進められている。その成功には,早期診断,早期介入が必要である。ALSを早期診断するためには,神経科学の知識と臨床の知識をつなぎ,本症に早く気付く必要がある。本症の筋力低下の分布には特徴があり,上腕が比較的スペアされ,split handがあれば強く疑う。電気生理学的には,経頭蓋磁気刺激法が上位運動ニューロン障害を捉える点で,神経反復刺激試験での僧帽筋の漸減現象は早期の運動神経障害を検出する点で期待される。血清,髄液中のリン酸化したニューロフィラメントは神経障害のマーカーとなる可能性がある。これらによる早期診断手法の開発が望まれる。
