Abstract

Vieillesse et mésusage d’alcool concentrent des stéréotypes négatifs, pouvant générer parmi les soignants un cumul de représentations défavorables à propos de sujets âgés avec trouble d’usage d’alcool, et induire leur négligence par contre-attitudes négatives. Objectifs : Approcher les attitudes soignantes par le biais de la forme des discours recueillis à propos d’aînés mésusant d’alcool. Méthodes : Une étude par entretiens individuels a concerné 45 soignants hospitaliers (69 % de femmes), parmi 582 infirmiers et 116 médecins de huit établissements de santé (hôpitaux ou établissements d’hébergement pour personnes âgées dépendantes) avec analyse textuelle non informatisée puis analyse automatisée du contenu des discours, par le logiciel Alceste®. Résultats : Si l’humour émerge spécifiquement de l’analyse textuelle non informatisée dès que les propos concernent les consommations d’alcool, une altération du choix des mots est rapidement apparue lors de l’analyse automatisée par Alceste®, avec recours à des termes familiers ou triviaux, jamais retrouvés dans aucune autre thématique des discours. Discussion : Ces constats ouvrent vers diverses hypothèses sous-tendant ces altérations du langage soignant entre : possible recherche de proximité, jeu sur la similitude-dissemblance, spécificité associée aux particularités issues de pathologies dites auto-infligées, forme de provocation masochiste, dégoût inspiré par ces patients, avant d’enfin envisager certains aspects, positifs en osant parler du plaisir à boire de l’alcool. Conclusion : Au-delà de l’apparence des discours, la prise en compte des éléments internes à la pensée des soignants permet d’aborder le comportement de ces professionnels. Cette approche vise à promouvoir un regard psychodynamique sur les personnes âgées mésusant d’alcool au travers des attitudes soignantes.
