Abstract
REZUMATIntroducere.Boala hemoroidală (BH) reprezintă una dintre cele mai frecvente afecţiuni anorectale, având o prevalenţă de 4,4% şi multiple implicaţii sociale, medicale şi economice.De aici, interesul şi permanenta tendinţă de modificare a atitudinii privind abordarea şi tratamentul acestei afecţiuni.Material şi metodă.Am studiat tratatele şi articolele publicate pornind de la tehnicile clasice de tratament al BH şi încercând să identificăm tendinţele de evoluţie a acestora, precum şi apariţia unor noi tehnici şi tehnologii terapeutice şi gradul de implementare a acestora.Concluzii.Există o tendinţă de promovare a tehnicilor ambulatorii în tratamentul BH, ele ajungând în unele ţări la 90% dintre cazuri.Medicina ambulatorie are multiple avantaje: creşte eficienţa utilizării paturilor de spital, creşte gradul de satisfacţie al pacientului, scade costurile per caz rezolvat.Intervenţiile moderne de cabinet (ligatura prin benzi elastice, scleroterapia, coagularea cu infraroşu) sau noile intervenţiile miniinvazive (DG-HAL, PPH) îndeplinesc condiţiile anestezice şi chirurgicale ale unei intervenţii de ambulator.O operaţie ideală ar trebui să aibă un număr cât mai mic de recidive, să nu se însoţească de complicaţii majore, să permită o reintegrare în activitate cât mai rapidă, să se poată efectua în condiţii de ambulator şi să implice costuri cât mai mici.Problema ghidurilor de bună practică în medicină a devenit din ce în ce mai actuală pe măsură ce a crescut importanţa practică a aplicării medicinei bazate pe dovezi, a cantităţii din ce în ce mai mari de informaţii medicale, a implicării conflictelor medico-legale în relaţia dintre medic şi pacient.Pornind de la aceste ghiduri, fiecare unitate sanitară ar trebui să aibă propriile protocoale şi proceduri.
