Nyeri persalinan adalah kondisi fisiologis yang secara umum akan dialami oleh hampir semua ibu bersalin.Nyeri yang terjadi dapat mempengaruhi kondisi ibu berupa kelelahan, rasa takut dan rasa khawatir dan kekurangan cairan yang akhirnya dapat mempengaruhi proses persalinan dan mempunyai efek yang kurang baik terhadap janin. Tujuan dari penelitian ini adalah untuk mengetahui perbedaan tingkat nyeri persalinan kala I fase aktif pada ibu primipara dengan ibu multipara di Klinik Siti Reswari Padang. Penelitian ini menggunakan desain penelitian observasional. Populasinya adalah ibu primipara dan ibu multipara kala I fase aktif di Klinik Siti Reswari Padang yang berjumlah 43 orang. Teknik sampel menggunakan acidental sampling. Pengukuran intensitas nyeri menggunakan skala analog visual dan dengan menggunakan kuesioner McGill., data dianalisa menggunakan uji-T dengan derajat kemaknaan 95%.Hasil penelitian didapat tingkat nyeri persalinan kala I fase aktif pada ibu primigravida sebagian besar mengalami nyeri berat sebanyak 16 orang (84,2%). Nyeri persalinan pada ibu multigravida lebih dari separoh mengalami nyeri sedang sebanyak 14 orang (58,3%). Hasil analsisis T-test terdapat perbedaan tingkat nyeri persalinan Kala I Fase Aktif Pada Primipara dengan multipara nilai nilai p-value 0,000. Penting memahami dan mengenal fisiologi nyeri dalam proses persalinan kala I dan II agar bidan dapat melakukan manajemen pengurangan nyeri persalinan yang sesuai dengan jalur fisiologi. Edukasi yang akurat tentang pengurangan rasa nyeri dalam persalinan harus di informasikan pada saat kehamilan bertujuan untuk meningkatkan kepercayaan diri wanita pada saat persalinan.
