CEFALÉIA E ALTERAÇÕES ESTOMATOGNÁTICAS EM PACIENTES APÓS CRANIOTOMIA: ESTUDO CLÍNICO E ALGOMÉTRICO

Abstract
Introducao: As craniotomias pteronais sao tecnicas de disseccao e reinsercao do musculo temporal, afetando a Articulacao temporomandibular. Objetivos: Avaliar clinicamente o sistema estomatognatico em pacientes submetidos a craniotomia pterional e a incidencia e impacto da cefaleia na vida dessas pessoas. Metodos: Estudo clinico quantitativo realizado de acordo com a Resolucao no 466, de 12 de dezembro de 2012, do Conselho Nacional de Saude (CNS), CAAE: 62911516.9.0000.5371, onde um total de 15 pacientes foram submetidos a craniotomia pterional e tiveram sua disfuncao temporomandibular (DTM) avaliada atraves dos criterios de diagnosticos de pesquisa em DTM, foi realizada a mensuracao do limiar de dor a pressao dos musculos mastigatorios, com aplicacao da versao brasileira do questionario McGill e teste de impacto de cefaleia versao 6 no pre-operatorio de 60, 120 e 180 dias apos a cirurgia. A analise estatistica foi realizada atraves do teste do qui-quadrado, ANOVA para medidas repetidas, seguida pelo pos-teste de Dunnet ou teste de Friedman seguido do pos-teste de Dunn de acordo com a necessidade. Resultados: O presente estudo demonstrou que, apesar da boa recuperacao pos-operatoria, a maioria dos individuos submetidos a craniotomia pterional teve a abertura da boca limitada ao longo da pesquisa. O numero de individuos que referiram dor a palpacao da articulacao temporomandibular aumentou significativamente aos 60 dias apos a cirurgia. Os limiares de dor a pressao para o musculo temporal anterior reduziu significativamente em 120 dias apos a cirurgia e o impacto da dor de cabeca diminuiu gradualmente durante o estudo. Conclusao: Pacientes submetidos a craniotomia pterional apresentaram piora na percepcao algica a palpacao no musculo temporal e da articulacao temporomandibular com limitacao da abertura da boca. No entanto, a cefaleia mostrou reducao gradual do seu impacto na vida dos entrevistados.
