L'accouchement s'est considérablement médicalisé au Québec depuis vingt ans. C'est d'ailleurs à ce phénomène, caractérisé par l'hospitalisation des accouchées et par l'usage accru de techniques obstétricales sophistiquées, que la profession médicale attribue la baisse de la mortalité maternelle et périnatale. Pourtant, des études montrent non seulement que cette prétention n'est pas fondée, mais qu'en plus, la médicalisation de l'accouchement comporte des risques pour la santé et le bien-être de la mère et de l'enfant. L'amélioration du pronostic en obstétrique serait plutôt attribuable à des facteurs sociodémographiques qu'à l'intervention médicale. C'est ce que tend à démontrer l'expérience par exemple des Pays-Bas où les accouchements à domicile sont très répandus et où les indices de mortalité sont meilleurs qu'au Québec. Des études américaines comparant les mérites respectifs de l'accouchement à l'hôpital et de l'accouchement à domicile vont aussi dans ce sens. Les obstacles à l'accouchement humanisé, moins technique et hors de l'hôpital seraient donc davantage de nature politique que scientifique ou médicale.
