Abstract
Türk Ortopedi ve Travmatoloji Birliği Derneği LUNATUM AVASKÜLER NEKROZU (KİENBÖCK HASTALIĞI)Lunatum avasküler nekrozu (lunatomalazi), ilk olarak radyolog Robert Kienböck tarafından 1910 yılında tanımlanmıştır.[1] Hulten ise 1928 yılında yaptığı çalışmada, lunatomalazi hastalarında normal popülasyondan yaklaşık üç kat fazla (%74) negatif ulnar varyans olduğunu, bu durumun lunatum üzerindeki stresi artırdığını ve lunatomalaziye neden olduğunu ileri sürmüştür.[2] Ancak yüz yılı aşkın süredir araştırılan bu hastalığın kesin nedeni halen tam olarak bilinmemektedir. Klinik ve SınıflandırmaKienböck hastalığı çoğunlukla 20-40 yaş arası erkekleri etkiler ve tipik olarak tek taraflı (unilateral) olarak görülür.[3,4] Hastaların başvuru şikâyetleri çoğunlukla dorsal el bilek ağrısı ve kavrama gücünde azalmadır.Bu durum karpal anatomide ilerleyici bir etkilenme olduğuna işaret eder.[4] A vasküler nekroz (osteonekroz, aseptik nekroz) kemik iliğinin ölümü ile sonuçlanan durumları ifade eder.El bileği kemikleri içerisinde en sık lunatum ve skafoid kemik, daha az sıklıkla kapitat ve pisiform kemik etkilenir.Lunatum avasküler nekrozu (lunatomalazi) Kienböck hastalığı ve skafoid avasküler nekrozu Preiser hastalığı olarak adlandırılır.Elde ise nadiren görülen metakarp başı avasküler nekrozu Dietrich hastalığı ve falanks avasküler nekrozu ise Thiemann hastalığı olarak adlandırılır.El ve el bileği avasküler nekrozlarının tedavileri yeni teknikler ve modifikasyonlar ile sürekli güncellenmektedir.Cerrahi tedaviler, kemikteki tutulum oranına, çökme evrelemesi ve derecesine göre değerlendirilmektedir.Bu tedavi prosedürleri, eklem seviyesini dengeleyen cerrahileri
