Abstract
背景與目的:普瑞德威利症候群(Prader-Willi syndrome)肇因於第15對染色體長臂基因缺陷,此個案在嬰幼兒時期典型症狀為低張力、吸吮能力不佳及全面性發展遲緩,而需接受物理治療介入。本研究旨在分析普瑞德威利症候群嬰幼兒接受早期物理治療介入之情形及其粗大動作發展狀況。方法:個案懷孕週數40週,自然產,生產過程無特殊狀況發生,經由小兒科醫生診斷為普瑞德威利症候群,於三個月大開始至復健科接受門診物理治療介入。經物理治療師評估後發現個案張力較低、運動耐力較差、餵食困難、粗大動作發展遲緩,給予粗大動作之誘發、頭部軀幹控制、功能性活動等訓練,家屬衛教則融合以家庭為中心概念,於個案每次治療時給予疾病相關諮詢,教導個案家屬相關治療手法和運動介入,並讓家屬於家中進行治療。粗大動作評估工具為阿爾伯塔嬰幼兒動作評估量表(Alberta Infant Motor Scales, AIMS),觀察個案粗大動作經過治療後整體發展。結果:個案於三個月時的評估結果阿爾伯塔嬰幼兒動作評估量表總分為5分:趴姿1分、躺姿3分、坐姿0分、站姿1分,其動作發展分數為低於同年齡5%以下,運動耐力持續時間約持續10 ~ 15分鐘,肢體呈現低張力。進行物理治療介入為每週2次,每次介入時間30分鐘,治療前後皆由專一物理治療師進行評估。經過三個月治療,目前個案年紀六個月大,阿爾伯塔嬰幼兒動作評估量表分數總分由5分進步至17分:趴姿5分、躺姿5分、坐姿4分、站姿1分,運動耐受時間可以持續約30分鐘,肢體張力相較於三個月大時略增,對於環境外界刺激反應亦較佳。三個月介入期間,家屬會根據在家中進行治療結果給予回饋及提問,而治療師給予相關建議和諮詢,家屬表示個案於家中的運動時間由一次15 ~ 20分鐘進步為一次30 ~ 40分鐘。與家屬適當溝通並訂定共同治療目標,使家屬心理壓力相對減少。結論:經由三個月早期物理治療介入和以家庭為中心概念介入普瑞德威利症候群嬰幼兒和其家屬,針對其粗大動作進行治療和諮詢,能改善個案先天張力較低和粗大動作發展遲緩問題,增加其運動整體耐受度,並降低家屬心理壓力。臨床意義:此個案於三個月大時,即接受物理治療早期介入,經持續介入有效改善個案張力過低、粗大動作發展遲緩和運動耐受度⋯⋯等問題,顯示早期介入針對普瑞德威利症候群個案,對於其症狀能有效的改善,再加上家庭為中心模式介入給予家屬適當諮詢和衛教,有助於個案動作發展和耐力,並降低家屬心理壓力。
