Evaluation of the performance and concordance of Fibroscan and the APRI score in patients with chronic hepatitis C

Abstract
Introduction. Plusieurs méthodes non invasives d’évaluation de la fibrose ont été développées qui ont prouvé leur intérêt en cas d’hépatite virale chronique C. L’objectif de cette étude est d’évaluer la performance diagnostique de la fibrose par le Fibroscan et le score APRI en les comparant à la biopsie hépatique chez les patients atteints d’hépatite virale chronique C dans la wilaya de Batna. Matériels et Méthodes. Une étude descriptive d’observation de 72 patients atteints d’hépatite C chronique, confirmés par une PCR, ont bénéficié d’une évaluation de la fibrose hépatique par le Fibroscan, le score APRI et la biopsie hépatique. Les valeurs de coefficient Kappa ont été utilisées pour évaluer le degré d’accord entre ces examens. La performance diagnostique de Fibroscan a été évaluée par la mesure des aires sous la courbe ROC (AUROC) et la valeur seuil optimale. Résultats. L’âge moyen de ces patients est de 56,35 ± 8,63 ans. Un sex-ratio (H/F) de 0,47. La moyenne de l’élasticité de ces patients est de 16,08 ± 9,99 Kpa, allant de 4,30 Kpa à 46,40 Kpa. Les résultats de l’analyse de la concordance ont montré un accord modéré significatif entre le Fibroscan et la PBH avec un Kappa à 0,597, P < 0,001. Les AUROC pour le Fibroscan/ le score APRI pour le diagnostic de fibrose significative (F ≥ 2), sévère (F ≥ 3) et de cirrhose (F = 4) sont 0,924 (0,863 - 0,986) / 0,710 (0,578- 0,842) ; 0,923 (0,848-0,999) / 0,804 (0,702- 0,906) ; 0,924 (0,855 - 0,992) / 0,878 (0,800 - 0,956) respectivement. Les seuils optimaux de l’élasticité pour la fibrose significative (F ≥ 2), sévère (F ≥ 3) et de cirrhose (F = 4) sont 9,9 Kpa, 11,6 Kpa, 15,3 Kpa respectivement. Conclusion. Dans notre série, le Fibroscan reste l’examen le plus performant dans le diagnostic de la fibrose hépatique quel que soit son degré.
