Development of an MRI-guided Robotic Platform and Optimized Treatment Parameters for Sonothrombolysis of Intraventricular Blood Clots

Abstract
Intraventricular haemorrhage develops in 3% of all live births in Canada and 50% of those babies develop post-haemorrhagic ventricular dilatation due to blood clots in the ventricles. This often leads to hydrocephalus, which can negatively impact an infant’s vision, hearing and long-term neurodevelopment. Current treatment involves invasive brain surgery that relieves the built-up pressure from hydrocephalus, but the clot is often left untreated. In this thesis, a robotically controlled focused ultrasound platform was evaluated as a method to perform sonothrombolysis of the clot. A range of focused ultrasound parameters were tested in vitro with porcine blood to determine what values effectively and efficiently induced sonothrombolysis. The robot’s current state was assessed and necessary changes to the hardware and software were implemented to bring the robot to a working condition. The thesis concludes with a robotically controlled focused ultrasound platform along with a set of optimized parameters recommended for sonothrombolysis.
