The role of bracing in adolescent idiopathic scoliosis (AIS) has been poorly defined, and controversy has persisted in regard to the efficacy of bracing. Initially designed solely as a randomized trial, the Bracing in Adolescent Idiopathic Scoliosis Trial (BRAIST) was conducted in 25 institutions across the United States and Canada. This trial was designed to investigate the efficacy of bracing compared to observation alone in preventing moderate AIS curves (20°-40°) from reaching 50° or more (i.e., surgical magnitude). However, many families ultimately refused randomization, and a preference cohort was subsequently added to the trial. Thus, a limitation was the lack of complete randomization. However, due to the nature of the treatment and high rate of parental and patient preference surrounding bracing, this issue was likely unavoidable. In addition, some patients with curves close to 50° may eventually require surgical intervention.
