Commentary on: Five-Year Safety and Satisfaction With the Lightweight Breast Implant

Abstract
It is with great pleasure that we discuss “Five-Year Safety and Satisfaction With the Lightweight Breast Implant” by Drs Govrin-Yehudain et al.1 In this article, the authors reviewed a 5-year, single-surgeon experience employing a new lightweight breast implant (LWBI) in primary and revision breast augmentation procedures. The B-Lite (G&G Biotechnology Ltd., Haifa, Israel and Polytech Health and Aesthetics GmbH, Dieburg, Germany) LWBI was designed to maintain the equivalent size, form, and function of traditional silicone gel breast implants while weighing approximately 30% less. The authors’ reasoning for employing this LWBI is based on fundamental laws of physics, where F = mg (F, force; m, mass; g, gravitational acceleration) and where the weight acting downwards on an object depends only on the mass of the object and the effect of gravity—volume is not a factor. Additionally, Hooke’s law states that the degree of tissue stretching will be proportional to breast and breast implant mass and inversely proportional to its stiffness (inelasticity). In a static upright posture, the weight of a breast implant will displace the breast downwards with a force proportional to the weight of the breast implant—the greater the weight, the greater the displacement. This effect is magnified in dynamic states. The effect of weight of a breast implant results in a permanent and longstanding stress causing continuous tissue creep deformation. If all other factors are equal, including the volume of the breast implant, then its weight is the single most dominant variable affecting long-term changes in the soft tissues of the breast.
