VP48.12: Vascular function in nulliparous women: a longitudinal study

Abstract
To estimate the changes in vascular function throughout pregnancy and their relationship with biochemical and placental ultrasonographic markers. Nulliparous women were enrolled at 11-14weeks of gestation. Vascular function was evaluated by flow-mediated dilation (FMD), hyperemic velocity-time integral (VTI) and peripheral arterial tonometry (PAT) in each trimester of pregnancy and 6-12 weeks postpartum. Uterine artery pulsatility index (UtA-PI) was measured at 11-14 weeks and 20-24 weeks, while soluble fms-like tyrosine kinase-1 (sFlt-1), placental growth factor (PlGF), sFlT-1/PlGF ratio and pregnancy associated plasma protein A (PAPP-A) were tested at 11-14 weeks. Skilings-Mack test followed by pairwise comparison, Pearson's correlation and Spearman's rank correlation tests were applied. Forty women (mean age, 32.1 ± 4.0 years) were recruited among which we observed: 1) No significant changes of FMD over pregnancy; 2) a decline of PAT index (first trimester: 1.96, 95% confidence interval [CI] 1.44-2.53 vs. third trimester 1.39 (95% CI 1.30-1.62) p < 0.0001) during pregnancy that recovered after delivery with an increase in the baseline pulse wave amplitude during pregnancy; 3) no significant change of hyperemic VTI but statistically significant increase of baseline flow and baseline VTI in the third trimester; 4) mild negative correlations between UtA-PI with baseline flow and VTI in the first trimester. There were few significant changes in vascular function over pregnancy with only a mild correlation with placental biomarkers. FMD, VTI and PAT index might not be directly used as markers to represent the macrovascular and microvascular function during pregnancy due to the systematic vasodilation.
