Mixed Adenocarcinoma in a 44-Year-Old Male with Crohn’s Disease

Abstract
We herein report an extremely rare case of concurrent adenocarcinoma and neuroendocrine carcinoma (NEC) of the small bowel presenting as intestinal obstruction and found incidentally at laparotomy in a 44-year-old male with long-standing ileal Crohn’s disease (CD). The 44-year-old male with CD was diagnosed at age 30; Montreal, Canada (classification code A2B2L1). Before his diagnosis, 4 prior admissions due to obstructive symptoms were reported (2, 3, 7, and 10 years earlier). He was treated with 150 mg of azathioprine daily and 350 mg of infliximab every 8 weeks, both of which were discontinued due to nonremission. Therefore, he started treatment with 80 mg subcutaneous (sc) adalimumab weekly with a poor response. He was admitted for abdominal pain without fever or diarrhea. Abdominal examination revealed mild right lower quadrant tenderness without rigidity, guarding, or masses. Initial blood work showed no findings of interest. Fecal calprotectin was 652 mg/g.
