• Primary plasma cell leukemia (pPCL) is a rare and aggressive malignancy with a distinct clinical presentation compared to multiple myeloma. • pPCL has preponderances to specific primary cytogenetic abnormalities such as t (11;14) as well as other high risk cytogenetic features. • Achieving a deep and sustained response is an ideal goal for improved outcomes in this aggressive malignancy. • Future clinical trials incorporating novel small molecule inhibitors and immunotherapeutic approaches into the treatment armamentarium are critical for improving the outcomes of patients with pPCL.
