Abstract
A síndrome da anorexia-caquexia (SAC) é uma complicação freqüente no paciente portador de uma neoplasia maligna em estado avançado. Caracteriza-se por um intenso consumo dos tecidos muscular e adiposo, com conseqüente perda involuntária de peso, além de anemia, astenia, balanço nitrogenado negativo, devido alterações fisiológicas, metabólicas e imunológicas. A SAC é intensificada pelas alterações no metabolismo dos nutrientes (carboidratos, proteínas e lipídios), alterações hormonais (leptina, NPY, MC, grelina), além do aumento das citocinas circulantes (TNFa, IL-1, IL-6, IFN). Mudanças na percepção de paladar e olfato ocorridas com a progressão tumoral e com o tratamento oncológico, também contribuem com a anorexia e, conseqüentemente, com a SAC. O tratamento nutricional é realizado com a utilização de nutrientes especiais, como os ácidos graxos polinsaturados EPA e DHA, os aminoácidos glutamina e arginina e os nucleotídeos. A nutrição deve ser seguida junto ao tratamento farmacológico, com estimulantes do apetite, devido maiores resultados positivos para o paciente com SAC.
