Ultrastructural analysis of neurons, glia and capillaries in the peri‐infarct zone of an ischemic stroke

Abstract
Stroke is a serious condition that can drastically affect sensation, mobility and/or cognitive function. The peri‐infarct zone (or penumbra) is a hypo‐perfused region that, if re‐perfused, can regain function. To better understand this zone, mice were subjected to photothrombotic stroke in the forelimb somatosensory cortex (SSC) and allowed to recover for three days. Mice were then perfused and the SSC processed for light and electron microscopy. Infarcts appeared as swollen pale regions of the SSC that ranged in volume from 2–3mm 3 . Three zones were identified: the necrotic ischemic core, edematous, and structurally‐intact zones. In the necrotic zone, scattered erythrocytes, macrophages, neutrophils and eosinophils occupied the degenerating neuropil. The edematous zone (~100μm wide, adjacent to necrotic zone) exhibited swollen axons/dendrites 1–5μm wide, enlarged pyramidal neurons, glycogen‐filled astrocytes and capillaries with thickened and vacuolated endothelium. The intact zone (~200μm wide) contained dilated vessels surrounded by astrocytes with vast glycogen deposits. Excitatory and inhibitory synapses appeared structurally normal but reduced in number in the latter two zones. Understanding the cellular effects that occur in this zone will permit a more directed approach to therapies designed to reverse this damage.
