視神経脊髄炎スペクトラム障害は,2005年に抗アクアポリン4 (AQP4)抗体の発見とともに疾患概念が確立した中枢神経の自己免疫疾患で,抗AQP 4 抗体による補体依存性のアストロサイト傷害を中心とした炎症が重度の神経症状を引き起こす。病理学的には壊死性変化を伴う組織破壊が認められ,広範なAQP 4の脱落や血管壁への補体や免疫グロブリンの沈着が認められる。診断には血清抗AQP 4 抗体の同定が必要十分条件であり,感度の高い検査法を用いるのが望ましい。
