Abstract
46歳男性．直腸癌，特発性血小板減少症に対して開腹高位前方切除術と脾臓摘出術を施行したが，術後の膵液瘻を認めた．膵管ステント留置等の治療により改善したが，後に膵液瘻の再発と腹腔内膿瘍を認めた．従来の治療では難治が予想され，ERPにて膵液瘻が描出されたため，経乳頭的にコイル塞栓術と膵管ステントを留置した．その後に経皮的にn butyl-2-cyanoacrylate（NBCA）の注入を行い，膵液瘻の上流·下流から挟み込むように塞栓を施行した．ERCP後膵炎を認めたが，保存的治療で軽快した．直腸癌の再発と，後に膵液瘻の再発が疑われ，手術となった．難治性の膵液瘻に対する治療法として有効性と安全性につき今後検討が必要である．
