A neurossífilis acomete o Sistema Nervoso Central e pode causar diversos prejuízos inclusive fonoaudiológicos, uma vez que pode comprometer a fala, a linguagem, praxias oro-faciais e até mesmo alimentares. O presente estudo tem como objetivo descrever um caso clínico de um jovem paciente portador de neurossífilis e as alterações encontradas nas avaliações fonoaudiológica e neuropsicológica. RSS, sexo masculino, 22 anos de idade, 11 anos de escolaridade, foi encaminhado para avaliação com histórico de parestesias, perda de força muscular e declínio cognitivo progressivo. Foi avaliado por meio do Mini Exame do Estado Mental, Bateria Montreal-Toulouse de Avaliação da Linguagem, ASHA-FACS, Protocolo de avaliação da Apraxia da Fala e Instrumento de Avaliação Neuropsicológica Breve NEUPSILIN. O paciente apresentou desempenho deficitário em todas as avaliações realizadas. Sendo diagnosticado com apraxia da fala, disartria hipocinética e alterações linguísticas e extralinguísticas. É importante que para o diagnóstico e planejamento terapêutico adequado se conheça todas as dificuldades dos quadros de alterações neurológicas.
