New onset myoclonus and encephalopathy in a woman with multiple sclerosis: Consider the medications

Abstract
A woman with multiple sclerosis (MS), diagnosed at 32 years old, developed episodes of encephalopathy and myoclonus at 46 years old that increased in frequency and severity over time. After extensive work-up for these symptoms in a complex MS patient, the most likely etiology was drug-induced as down-titrating medications originally initiated for MS-associated symptoms (baclofen, tiagabine) significantly improved her encephalopathy and myoclonus. This case demonstrates that new symptoms in a known neurologic disease require careful analysis of temporality and characteristics to elucidate the etiology, particularly when unusual for the underlying condition, and additional medications may worsen rather than alleviate symptoms.
