Abstract
Temos assistido a um incremento da complexidade dos problemas de saúde, associado, entre outros, ao envelhecimento populacional e à multimorbilidade e dependência, mas também a uma consciência mais aguda do acesso à saúde enquanto direito. Por múltiplas razões, os sistemas de saúde não têm sido muito ágeis a responder a esta complexidade. Urge a discussão estratégica desta problemática, bem como o desenvolvimento de experiências inovadoras, as quais, devidamente monitorizadas, nos permitirão vislumbrar diferentes formas de alcançar o mesmo objetivo: proporcionar cuidados de qualidade, que contribuam para a saúde e bem-estar das pessoas. Neste contexto, apresenta-se uma proposta baseada em robusta base empírica, que assume como desiderato a domiciliação de cuidados, integrados num percurso de cuidados centrados na pessoa e no seu contexto familiar, e que garante a integração e a continuidade de cuidados. Por obediência aos mesmos princípios, propõe ainda uma redefinição do modelo de cuidados a pessoas dependentes institucionalizadas em respostas da rede social.
