Abstract
This chapter presents a case scenario of a 63-year-old woman who is otherwise independent who presents with progressive, symptomatic end-stage tricompartmental osteoarthritis of the knee. She has exhausted nonoperative modalities and wishes to proceed with total knee arthroplasty (TKA). Primary TKA implant designs differ in terms of treatment of posterior cruciate ligament. Considering the marked health economic burden of TKA, and the notable patient dissatisfaction rate with this procedure, it is critical that evidence-based decisions be made around implant selection so as to maximize healthcare value. In terms of patient-reported outcomes of TKA, the best available evidence suggests no difference in outcomes associated with the use of cruciate-retaining (CR) versus posterior stabilized designs. The use of CR TKA implants may be associated with a small improvement in implant survival as compared to PS designs, although the quality of evidence supporting this is low. The chapter provides recommendations for implementing evidence-based practice in the clinical setting.
