This chapter presents a case scenario of a 54-year-old male who presents to clinic six months after right primary total knee arthroplasty (TKA). Identification of the microbial pathogen(s) is essential for diagnosis of TKA infection and for selection of appropriate pathogen-specific antimicrobial therapy. The initial history and physical exam are crucial to diagnosis of periprosthetic joint infection. Erythrocyte sedimentation rate and C-reactive protein have excellent diagnostic test performance for chronic TKA infection and should be used as initial screening tools and adjuncts to synovial fluid cytology and culture. Infection is the leading indication for revision surgery after TKA, and yet there is no gold standard for diagnosis. TKA infection can be a devastating complication and is associated with significant morbidity to patients and tremendous costs to the healthcare system. Knee arthrodesis and above-knee amputation are salvage procedures in multiply failed revision TKA patients. The chapter provides recommendations for implementing evidence-based practice in the clinical setting.
