This chapter presents a case scenario of a 53-year-old male who presents to an ambulatory clinic with a history of bilateral hand numbness and clumsiness, leading to great difficulty with manual tasks, as well as gait difficulty. The spectrum of disease severity is wide in degenerative cervical myelopathy (DCM), and the severity of initial presentation dictates the optimal clinical management of affected patients. DCM occurs when progressive age-related osteoarthritic changes narrow the cervical spinal canal, leading to chronic spinal cord compression. Many patients present with imaging evidence of cervical spinal cord compression, but without clinical signs or symptoms of DCM or radiculopathy. The goal of surgery is to provide decompression of the cervical spinal cord, which can be achieved through an anterior, or combined approach, followed by reconstruction and stabilization of spinal column to address any element of instability, whether degenerative or iatrogenic. The chapter provides recommendations for implementing evidence-based practice in the clinical setting.
