Resume Objectif : Les maladies chroniques representent un fardeau croissant pour les systemes de soins des pays developpes. Pour faire face a leur nature multifactorielle, des modeles integres de prevention et de gestion ont ete suggeres. Bien que ces modeles aient demontre qu’ils ameliorent les resultats des soins des personnes atteintes de maladies chroniques, leur implantation a l’echelle des systemes de sante demeure un defi. Cette etude avait pour objectif d’identifier les facteurs qui faciliteraient ou nuiraient a l’implantation de ces modeles de soins.Methodes : Etude de cas du systeme de soins du Quebec. Des entrevues semi-structurees d’experts et de decideurs ont ete realisees en employant une grille d’entrevue. Une revue exhaustive de la litterature et une analyse de contenu ont ete realisees en employant une grille standardisee. Les resultats des entrevues et de la recension des ecrits ont ete triangules a l’aide d’un cadre d’analyse.Resultats : Le manque d’outils de gouverne clinique, une faible infrastructure d’information clinique, des plans de remuneration inappropries, une faible couverture publique des services multidisciplinaires et des deficits a l’organisation des services de sante ont ete rapportes comme des barrieres. L’emergence de nouveaux modeles de pratique de groupe en premiere ligne, une integration croissante des etablissements du reseau, un systeme communautaire de services sociaux developpes et un champs de la sante publique developpe ont ete percus comme des elements facilitants.Conclusion : Divers facteurs sont identifies comme facilitant ou limitant l’implantation de modeles integres de soins aux maladies chroniques. Une attention devrait etre portee aux mecanismes de gouverne, aux modes de remuneration et de prestation des soins pour accroitre les chances de succes de leur implantation.Prat Organ Soins 2009;40(4):251-265
