Abstract
Ce travail a evalue la toxicite et l'efficacite de la radioimmunotherapie (RIT) en deux temps avec l'anticorps bispecifique murin anti-CEA/anti-DTPA F6-734 et le haptene bivalent di-DTPA-131I (reactifs AES) chez des souris greffees avec un cancer medullaire de la thyroide (CMT) et chez 26 patients porteurs de recidives de CMT inclus dans une etude de phase I/II. Chez l'animal, la biodistribution des reactifs AES etait plus favorable que celle du meme anticorps anti-ACE directement marque et la RIT en deux temps aussi efficace et moins toxique que la RIT en un temps. Des injections repetees de reactifs AES ou l'association avec le paclitaxel ameliorait l'efficacite de la RIT sans alterer la tolerance, l'association ayant un effet synergique. Dans l'etude clinique, la toxicite hematologique a limite l'escalade de dose. La dose maximale toleree etait de 48 mCi/m2. Les resultats dosimetriques etaient favorables et quatre reponses tumorales ont ete observees.
