Abstract
Dear Editor, We have had the opportunity to read the article by Langlois et al, “Hypophysitis, the Growing Spectrum of a Rare Pituitary Disease” (1), that was recently published in the Journal of Clinical Endocrinology and Metabolism. While the review is interesting, it has minor and major flaws that we wish to bring to your attention. An omission is the discussion of xanthomatous hypophysitis that is often associated with rupture of a Rathke cleft cyst. This is a distinct entity with a characteristic presentation clinically and radiologically. A major problem is the inclusion of the histiocytoses, including Langerhans cell histiocytosis and Erdheim-Chester disease. These are neoplastic entities (2, 3) and should not be considered in the spectrum of inflammatory disorders, as their treatment and prognosis can be quite different. The authors have nothing to disclose.
