Abstract
This chapter presents a clinical scenario of a 13-year-old girl who has activity-related pain below the medial malleolus, and on occasion in the medial arch of foot and in the sinus tarsi. X-rays are suggestive of a tarsal coalition with flatfoot deformity including 16° of hindfoot valgus. There are no degenerative changes of the posterior facet. Concomitant flatfoot deformity is commonly seen with talocalcaneal (TC) coalitions and can cause pain and disability. TC coalitions typically involve the middle facet, but can occur at any location in the subtalar joint. CN is the most common type of tarsal coalition, representing 54% of all coalitions. Although deformity is more common in TC coalitions, CN coalitions can also present with flatfoot deformity causing pain and disability. Unlike TC coalitions, CN coalitions do not involve a joint. They act as an extra-articular tether, limiting movement through otherwise healthy cartilage.
