Abstract
Over the past 15 years islet transplantation has emerged as an effective therapy for selected patients with brittle, refractory type 1 diabetes. Numerous changes have been made to improve the success rate of islet transplantation. We review the successes and pitfalls of islet transplantation, including its impact on diabetes-related complications and the acute and long-term effects of immunosuppression. Recent advances in the field of islet transplantation that have led to improved islet graft function and durability of graft survival are discussed. Important future directions for the field are also explored.
