Diagnosis: Sydenham chorea. The diagnosis of Sydenham chorea was suggested by the insidious development of choreic movements following an upper respiratory tract infection (figure 1; see also video 1, available in the electronic edition of Clinical Infectious DiseasesRecurrent arthralgia and the documentation of valvular regurgitation further supported a rheumatologic etiology. Culture of a throat swab yielded group A streptococci (GAS) with M protein type 5, a strain that has been identified as an etiologic agent in cases of acute rheumatic fever (ARF) [1]. Evidence of a recent GAS infection was confirmed by an elevated antistreptolysin O (ASO) titer of 1:795 (normal titer, <1:166). The hallmark characteristics of this manifestation of ARF are choreic movements, emotional lability, and hypotonia that may compromise the ability to sit or stand, as observed of our patient. Chorea often initially presents asymmetrically but typically progresses to bilateral involvement. Motor impersistence of the tongue and hands may limit independent feeding as well as hand use.
