Abstract
Peptic ulcers are localized defects of the gastrointestinal mucosa, most commonly found in the duodenum or stomach. Worldwide, peptic ulcer disease (PUD) and its complications remain the cause of much suffering and significant mortality. Duodenal ulcers typically occur in the duodenal bulb and are usually attributable to H. pylori ; if more distal duodenal ulcers are found, other etiologies, such as nonsteroidal antiinflammatory drugs (NSAIDs) or gastrinoma, should be considered. The epidemiology of PUD has changed markedly over the last century due to changes in the epidemiology of its environmental risk factors, including H. pylori and NSAID/aspirin use. In H. pylori -associated DUs, acid production is modestly increased and contributes to pathogenesis. The natural history of PUD depends on the underlying cause, but essentially ulcers may remit (even without antisecretory treatment), persist, or develop complications such as bleeding or perforation.
