P247. Unanticipated small bowel Crohn’s disease in elderly patients revealed by video capsule endoscopy

Abstract
Background: Inflammatory bowel disease (IBD) is highly associated with difficulties in work productivity, poor health-related quality of life, and activity impairment.The aims of this study were to quantify work limitations and activity impairment, as well as the effect on quality of life, in an IBD Spanish population.Methods: The study was designed as a cross-sectional study of outpatients referred from IBD multidisciplinary hospital units.Ademographic and clinical questionnaire was completed.Data included in the current analysis were collected for 7 months.Quality of life was assessed with the Inflammatory Bowel Disease Questionnaire (IBDQ-36), and work-related outcome was assessed with the Work Productivity and Activity Impairment (WPAI).The WPAI consists of 6 items that can be used to measure the effect of a person's specific health problem on work productivity and impairment in daily non-work-related activities during the preceding 7 days.Results: In total, 127 patients with IBD participated.The average age was 39 years, and the mean duration of disease in years was 9.9.Of the included patients, 53.5% were employed, and 22% were unemployed.Of the patients who were employed, the average of lost hours during the last week because of their health problem was 1.63.Further, 42% of their work was impaired, and 52% of regular daily activities had been limited.There were significant correlations amongst work-productivity impairment and activity impairment with quality of life (r = -.467 to -.626).In addition, limitations on daily activities predict quality of life (β = -.411,t = -2.99,p = 0.004).There were significant differences between males and females regarding work-productivity impairment and daily activities limitations, which was higher in males t (125) = -2.442,p = 0.016, and in quality of life t (125) = 4.04, p = 0.000.Overall, there were no differences between Crohn's and ulcerative colitis.Conclusions: Patients with IBD have to cope with work-productivity limitations and activities difficulties daily.This has a negative effect on quality of life, as well.The chronic course of IBD with all its restrictions to normal life may itself be a very stressful factor.In clinical practice, it should be kept in mind that IBD has a global effect on all important life dimensions of the person, and it requires a multidimensional approach.P247
