Когнитивные нарушения у больных хроническими вирусными гепатитами могут развиваться в разное время после инфицирования, в связи с их прямым и косвенным нейротоксическим действием на головной мозг. У 50% инфицированных хроническим вирусным гепатитом С могут обнаруживаться как клинические, так и субклиничекие проявления когнитивной дисфункции. Своевременная диагностика когнитивных нарушений у пациентов с хроническими вирусными гепатитами может служить показанием для своевременных лечебных мероприятий, направленных на улучшение качества жизни у данной категории больных. В данной статье рассматриваются когнитивные нарушения при хронических вирусных гепатитах у больных в Туркестанской области. Цель. Изучить качественную и количественную характеристику когнитивных функций при хронических вирусных гепатитах, с оценкой взаимосвязи выраженности когнитивных нарушений между стадиями фиброза печени и давностью заболевания у больных, состоящих на диспансерном учете в Областном гепатологическом центре г. Шымкент. Материалы и методы. Было обследовано 149 пациентов с хроническими вирусными гепатитами, состоящих на диспансерном учете в Областном гепатологическом центре г. Шымкент. Анализ когнитивного статуса проведен с применением Монреальской шкалы когнитивных функций МоСа. Были использованы описательные статистики, статистический критерий Шапиро-Уилка, корреляционный анализ с применением непараметрического критерия Спирмена для двух линейных переменных. Результаты и обсуждение. Результаты корреляционного анализа продемонстрировали наличие слабой отрицательной связи (r=-0,43, p < 0,04) между показателями МоСа и стадией фиброза печени, что указывает на умеренные нарушения когнитивных функций при хронических вирусных гепатитах. Заключение. У больных хроническими вирусными гепатитами на различных стадиях фиброза были в большинстве случаев, нарушением со стороны зрительно-конструктивных и исполнительных навыков, внимания, краткосрочной памяти, абстрактного мышления. Установление когнитивной дисфункции различной степени выраженности, в частности на ранней стадии у пациентов с хроническими вирусными гепатитами способствует проведению своевременных лечебных мероприятий, направленных на улучшение качества жизни. Timely diagnosis of cognitive disorders in patients with chronic viral hepatitis can serve as an indication for timely therapeutic measures aimed at improving the quality of life in this category of patients. This article discusses cognitive disorders in patients with chronic viral hepatitis in the Turkestan region. Goal. To study the qualitative and quantitative characteristics of cognitive functions in chronic viral hepatitis, with an assessment of the relationship between the severity of cognitive disorders between the stages of liver fibrosis and the duration of the disease, which are registered in the Regional Hepatological Center of Shymkent. Materials and methods. 149 patients with chronic viral hepatitis registered at the Regional Hepatological Center in Shymkent were examined. The analysis of cognitive status was carried out using the Montreal Scale of Cognitive Functions of the MoCa. We used descriptive statistics, the Shapiro-Wilk statistical test, and correlation analysis using the nonparametric Spearman test for two linear variables. Results and discussion. The results of the correlation analysis showed a weak negative relationship (r=-0.43, p < 0.04) between the MoCa parameters and the stage of liver fibrosis, which indicates moderate cognitive impairment in chronic viral hepatitis. Conclusion. In patients with chronic viral hepatitis at various stages of fibrosis, in most cases, visual-constructive and executive skills, attention, short-term memory, and abstract thinking were impaired. The establishment of cognitive dysfunction of various degrees of severity, in particular at an early stage in patients with chronic viral hepatitis, contributes to the timely implementation of therapeutic measures aimed at improving the quality of life.
