Abstract
This chapter highlights the similarities in chemical structure, and physiologic effects of opioids, as well as their therapeutic uses and potential for misuse or abuse. Special attention is given to the testing process, with an emphasis on interpretation of test results. The opioids are a diverse group of compounds that have in common the ability to bind and activate opioid receptors. Opioids exert their effects by controlling the movement of calcium and potassium ions across the cell membrane. Opioid use has a major impact on the US health-care system. Heroin is a semisynthetic opioid with 2–5 times the potency of morphine. Hydromorphone and oxymorphone are indicated in the treatment of moderate to severe pain. Fentanyl is a versatile drug that is used in many medical applications. Screening tests for opioids are usually performed by immunoassays with commercial reagents.
