Abstract
Testing for drugs and toxins can be performed on a variety of specimens. Choosing the most appropriate one depends on the purpose of the analysis. Blood is the specimen of choice for assessing the amount of active drug in a patient. Whole blood is required for certain analyses, because the drug or toxin of interest accumulates within red blood cells. This chapter discusses the issue of specimen validity testing. The window of detection is several hours to several days following drug use, depending on the specific drug and the amount and frequency of consumption. Many people are required to undergo drug testing as a condition of employment or to remain in drug rehabilitation programs. Specimen tampering can be divided into three categories: adulteration, substitution, and dilution. Dilute and substituted specimens are identified by comparing the values for creatinine and specific gravity.
