Contexte: L’utilisation du cannabis est en hausse chez les jeunes adultes, alors que ses effets sur la santé cardiovasculaire sont peu compris. Nous avons voulu évaluer le lien entre la consommation récente de cannabis et l’infarctus du myocarde (IM) chez les jeunes adultes (18–44 ans). Méthodes: Nous avons procédé à une étude transversale en utilisant les données regroupées des cohortes 2017 et 2018 de l’enquête américaine BRFSS (Behavioral Risk Factor Surveillance System) menée auprès des adultes américains. Nous avons analysé le lien entre la consommation récente de cannabis et l’IM à l’aide d’un modèle de régression logistique pondéré qui tenait compte de facteurs démographiques et socioéconomiques, de l’hygiène de vie, de l’utilisation concomitante de substances psychoactives, et d’autres comorbidités. Nous avons aussi évalué ce lien après stratification en fonction de la fréquence et du principal mode de consommation du cannabis. Résultats: Parmi 33 173 jeunes adultes (18,5 millions pondérés), 4610 répondants (3,2 millions pondérés) ont déclaré avoir récemment consommé du cannabis (17,5 %, intervalle de confiance [IC] à 95 %, 16,8 %–18,2 %). L’IM a été plus fréquent chez les récents utilisateurs de cannabis (n = 61 sur 4610, 1,3 %) que chez les non-utilisateurs (n = 240 sur 28 563, 0,8 %, rapport des cotes [RC] ajusté, 2,07, IC à 95 %, 1,12–3,82) et il a été associé à une consommation de cannabis plus de 4 fois par mois (RC ajusté, 2,31, IC à 95 %, 1,18–4,50) et à l’inhalation (combustion, vaporisation) comme principal mode de consommation (RC ajusté 2,01, IC à 95 %, 1,02–3,98). Interprétation: Notre étude vient étayer l’hypothèse d’un lien entre la consommation récente de cannabis et l’IM chez les jeunes adultes. L’utilisation croissante du cannabis chez une population à risque pourrait avoir des répercussions négatives sur la santé cardiovasculaire.
