Atopic Dermatitis: Psychological and Neuropsychological Characteristics and their Association with Clinical Features

Abstract
Введение. Стресс считается одной из причин обострения атопического дерматита. Способность понимать эмоции, способ совладания со стрессом, механизмы психологических защит, стрессоустойчивость и особенности исполнительских функций тесно связаны с субъективным восприятием стресса и индивидуальной способностью справляться со стрессом.Цель. Изучить степень выраженности психологических и нейропсихологических параметров относительно клинических проявлений атопического дерматита.Материалы и методы. На базе УЗ «Городской клинический кожно-венерологический диспансер» (переименован в сентябре 2021 г. в УЗ «Минский городской клинический центр дерматовенерологии») обследовано 59 пациентов с атопическим дерматитом. Для исследования использовались: анамнестический метод, визуально-аналоговая шкала интенсивности зуда, психологические методы (Торонтская алекситимическая шкала, TAS-20-R; опросник Дж. Маера, П. Сэловея и Д. Карузо; опросник Плутчика – Келлермана – Контэ; опросник Холмса – Раге; опросник способов совладающего поведения по Лазарусу; опросник соматизированных расстройств) и нейропсихологический метод (компьютерная версия Висконсинского теста сортировки карточек).Результаты. Исследование показало, что низкая эмоциональная осведомленность, дезадаптивные механизмы психологической защиты и копинг-стратегии, а также значительная выраженность механизмов соматизации, низкая стрессоустойчивость и нарушение исполнительских функций ассоциированы с более тяжелыми клиническими проявлениями атопического дерматита.Заключение. Полученные данные говорят о том, что в процессе обследования и лечения пациента с атопическим дерматитом вышеописанные параметры могут стать потенциальной мишенью для психологической коррекции. Это в будущем, возможно, сможет помочь снизить тяжесть клинического проявления данной патологии кожи. Introduction. Stress is considered to be one of the causes of atopic dermatitis. The ability to understand feelings and emotions, coping strategies, psychological defense mechanisms, stress resistance and executive functions are closely related with the subjective perception of stress and the individual’s ability to cope with stress.Purpose. To study the degree of expression of psychological and neuropsychological characteristics in relation to clinical features of atopic dermatitis.Materials and methods. 59 patients with atopic dermatitis were enrolled in the study at "The City Clinical Dermatovenerologic Dispancery" (renamed from September, 2021: "Minsk City Clinical Center of Dermatovenerology"). Research methods: anamnesis, Visual Analogue Scale, Toronto Alexithymia Scale-20-R, The Mayer – Salovey – Caruso Emotional Intelligence Test, Life Style Index, Social Readjustment Rating Scale, The Ways of Coping Check list, Screening for Somatoform Symptoms and computer version of the Wisconsin Card Sorting Test.Results. Low emotional intelligence, maladaptive coping strategies, primitive psychological defense mechanisms, somatization, low stress resistance and impaired executive functions are increasing the chances of developing more severe clinical picture in patients with atopic dermatitis.Conclusions. The results obtained indicate that in the process of examining and treating a patient with atopic dermatitis emotional awareness, coping strategies, psychological defense mechanisms, stress resistance and executive functions may become one of the main targets for psychological help in order to reduce the severity of the clinical manifestation for patients with atopic dermatitis.
