Influence of Sleep Apnea on Serum Superoxide Dismuthase Activity in Patients in the Acute Period of Cerebral Infarction

Цель. Изучить активность супероксиддисмутазы сыворотки крови, а также оценить ее диагностическую эффективность в качестве прогностического маркера неврологических, функциональных и когнитивных нарушений у пациентов с апноэ во сне в остром периоде инфаркта мозга.Материалы и методы. Было обследовано 90 пациентов с инфарктом мозга (ИМ) иапноэ во сне (АС) (основная группа) и 40 пациентов с ИМ без АС (группа сравнения). Обследование включало динамическую оценку по шкалам NIHSS, mRs, MOCA, исследование активности супероксиддисмутазы (СОД) сыворотки крови (в первые 72 часа и через месяц от начала ИМ), респираторную полиграфию или автономную полисомнографию. В основной группе 20 пациентам проводилось лечение АС методом СИПАП-терапии.Результаты. У пациентов основной группы в начале и конце острого периода ИМ наблюдались более низкие показатели антиоксидантной активности, чем у пациентов группы сравнения (р<0,001), при этом наименьшая активность СОД наблюдалась у пациентов с тяжелой степенью АС (р<0,001). У пациентов, получавших СИПАП-терапию, медиана СОД через месяц была значимо выше, в сравнении с пациентами без СИПАП-терапии (р=0,021), при этом исходно пациенты обеих подгрупп по уровню активности СОД не различались (р=0,131). У пациентов основной группы в качестве эффективных прогностических маркеров неблагоприятного функционального исхода (MRs ≥3) в конце острого периода ИМ установлены пороговые значения активности СОД в первые 72 часа от начала ИМ ≤35,6 единицы активности (е. а.) (AUC – 67,8%); выраженного неврологического дефицита (NIHSS >4) – ≤46,6 е. а. (AUC – 63,9%); наличия когнитивных нарушений (MOCA <26) – ≤45,8 е. а. (AUC – 81,1%).Выводы. Наличие АС, особенно его тяжелой степени, у пациентов в остром периодеИМ ассоциировано со снижением активности СОД сыворотки крови, в сравнении с пациентами без АС (р<0,05). У пациентов с АС пороговые значения активности СОД в первые 72 часа ИМ могут являться прогностическими маркерами выраженных неврологических и функциональных нарушений, наличия когнитивной дисфункции в конце острого периода ИМ. СИПАП-терапия АС у пациентов с ИМ способствует увеличению активности СОД в остром периоде заболевания. Purpose. To study the activity of blood serum superoxide dismutase, as well as to evaluate its diagnostic efficiency as a prognostic marker of neurological, functional and cognitive disorders in patients with sleep apnea in the acute period of cerebral infarction.Materials and methods. 90 patients with cerebral infarction (CI) and sleep apnea (SA) (main group) and 40 patients with CI without SA (comparison group) were examined. The examination included a dynamic assessment on the NIHSS, mRs, MOCA scales, a study of serum superoxide dismutase (SOD) activity (in the first 72 hours and one month after the onset of CI), respiratory polygraphy or autonomous polysomnography. In the main group, 20 patients underwent SA treatment by CPAP therapy.Results. In patients of the main group, at the beginning and at the end of the acute period of CI, lower indicators of antioxidant activity were observed than in patients of the comparison group (p<0,001), while the lowest SOD activity was observed in patients with severe SA (p<0,001). In patients who received CPAP therapy, the median SOD after a month was significantly higher compared with patients without CPAP therapy. (p=0,021), while the SOD activity of the patients of both subgroups did not differ at baseline (p=0,131). In patients of the main group threshold values of SOD activity in the first 72 hours after the onset of CI ≤35.6 activity units (a.u.) were set as effective prognostic marker of an unfavorable functional outcome (MRs≥3) at the end of the acute period of CI (AUC – 67.8%); ≤46.6 a.u. – of severe neurological deficit (NIHSS >4) (AUC – 63.9%); ≤45.8 a.u. – of the presence of cognitive impairments (MOCA <26) (AUC – 81.1%).Conclusions. The presence of SA, especially its severe form, in patients in the acute period of CI is associated with a decrease in the SOD activity of blood serum, in comparison with patients without SA (p<0,05). In patients with SA, threshold values of SOD activity in the first 72 hours of CI can be prognostic markers of severe neurological and functional impairments, the presence of cognitive dysfunction at the end of the acute period of CI. CPAP-therapy of SA in patients with CI increases the SOD activity in the acute period of the disease.
