Ein fallbasierter Ansatz für neue Richtungen in der Ernährungstherapie bei Morbus Crohn: ein Denkanstoß

Abstract
Neuere Evidenz deutet darauf hin, dass Morbus Crohn aus einer Dysbiose des Mikrobioms und anderen Umweltfaktoren herrühren könnte. Einer der stärksten Risikofaktoren im Zusammenhang mit der Immunaktivierung scheint in der Ernährung zu liegen. Es hat sich gezeigt, dass Diäten bei 70 bis 80% der therapienaiven Kinder zu Krankheitsbeginn Entzündungen lindern und eine Remission induzieren, und bei Patienten, die auf die derzeit empfohlene medizinische Therapie nicht mehr ansprechen oder ein Rezidiv entwickelt haben, eine Remission induzieren. Jüngste Studien brachten auch die diätetische Modulation des Mikrobioms mit der klinischen Remission in Verbindung, während die Wiederaufnahme der vorher gewohnten Diät zu einer Reaktivierung der Entzündungsreaktion und zu einer Reversion der Dysbiose führte. Während die Ernährungstherapie in der Regel im Rahmen einer Erstlinientherapie als Brücke zu Immunmodulatoren Anwendung fand, deuten neuere Erkenntnisse darauf hin, dass neue Behandlungsparadigmen, die eine Ernährungstherapie beinhalten, unterschiedliche Behandlungsstrategien ermöglichen könnten. In dieser fallbasierten Analyse der Fachliteratur wird die Morbus-Crohn-Ausschlussdiät (Crohn’s disease exclusion diet, CDED) als Monotherapie, Kombinationstherapie mit Medikamenten, als Rescue-Behandlung bei Patienten mit refraktärer Erkrankung und als Behandlung mit dem Ziel der Reduzierung der medikamentösen Therapie diskutiert.
