Abstract
Objetivo: O vegetarianismo é cada vez mais comum e transversal a todas as idades, sendo que uma dieta vegetariana nas crianças requer especial atenção por forma a suprir as exigências nutricionais necessárias ao seu desenvolvimento e crescimento. O objetivo deste trabalho é rever a evidência sobre alimentação vegetariana nas crianças, particularmente da diversificação alimentar aos primeiros anos de vida. Fontes de dados: National Guideline Clearinghouse, NICE, Canadian Medical Association Infobase, The Cochrane Library, DARE, MEDLINE, TRIP Database, UpToDate, Índex das Revistas Médicas Portuguesas e Direção-Geral da Saúde. Métodos: Pesquisa de normas de orientação clínica, revisões clássicas, revisões sistemáticas, meta-análises e artigos originais, utilizando os termos MeSH “diet, vegetarian”, “child” e “infant”, publicados em inglês ou português até junho de 2020. Resultados: Até aos seis meses é recomendado o aleitamento materno exclusivo. Nos lactentes vegetarianos, o aleitamento materno é aconselhado até aos dois anos. Se necessário, deve utilizar-se uma fórmula infantil de proteína de soja ou arroz para os lactentes saudáveis com dieta vegana. O tofu fresco natural pode iniciar-se aos seis a sete meses. Deve-se ter em conta a disponibilidade de proteína, ácidos gordos essenciais, vitamina B12 e D, iodo, ferro, cálcio, zinco e fibra, bem como o aporte energético da dieta. As crianças vegetarianas com uma dieta ajustada têm menor risco de obesidade, menor exposição a antibióticos presentes em produtos de origem animal e um melhor perfil de adipocinas anti-inflamatórias. Conclusões: Uma dieta equilibrada é fulcral para uma adequação nutricional e, ulteriormente, para um crescimento harmonioso. Embora a dieta ovolactovegetariana deva ser preferida em relação às outras dietas vegetarianas, o essencial é ter atenção à biodisponibilidade de nutrientes-chave e ao aporte energético. Assim, uma dieta vegetariana bem estruturada pode dar resposta às exigências nutricionais em idade pediátrica. O desenvolvimento adequado é preditor de uma dieta saudável.
