Las fístulas de las arterias coronarias constituyen una malformación vascular poco común. Debido a que la mayoría de los pacientes permanecen asintomáticos, aproximadamente la mitad de los casos pueden ser indetectables desde el punto de vista clínico. Reportamos el caso de un hombre de 61 años con una fístula del tronco coronario izquierdo a la vena cava superior, detectada incidentalmente, en el contexto de un infarto agudo de miocardio.
