b Bei einem Patienten mit Fieber und Haut- ausschlag sollte aufgrund der Anamnese und des klinischen Bildes schnell entschieden werden, ob der Patient rasch eine empirische antibiotische Therapie oder eine Isolation bei Verdacht z.B. auf hämorrhagisches Fieber braucht (Reiseanamnese!).b Aufgrund der typischen Hautläsion und des Alters des Patienten kann die Differentialdiagnose bereits eingeschränkt werden.Ebenfalls wichtig sind der primäre Ort des Hautausschlages und die Ausbreitung.So beginnt der Ausschlag bei Masern typischerweise retroaurikulär und breitet sich über Stamm und Extremitäten aus, und der Hautausschlag ist ein makulopapulöses Exanthem.Dagegen sind vesikuläre Exantheme eher mit Varizellen/Zoster-Viren vergesellschaftet.b Die Reiseanamnese ist äusserst wichtig, da z.B. das Dengue-Fieber am häufigsten bei Patienten nach Rückkehr aus Südostasien vorkommt, während «African tick-bite fever» nur in Afrika südlich der Sahara auftritt.* CME zu diesem Artikel finden Sie auf S. 590 oder im Internet unter www.smf-cme.ch* Vous trouverez les questions à choix multiple concernant cet article à la page 591 ou sur internet sous www.smf-cme.
