Glucose And Insulin Responses In Women At Risk For GDM Before And After A Nutrition, Exercise & Lifestyle Intervention Program (NELIP)

Abstract
Women with a pre-pregnancy BMI ≥25 are at increased risk for developing gestational diabetes (GDM). Prevention of GDM is important for both mother and fetus because of the etiology of prolonged high maternal blood glucose. PURPOSE To examine the effects of NELIP on glucose and insulin responses to a fasted oral glucose tolerance test (OGTT) in women at risk for GDM and to compare these results to reference values from women diagnosed with GDM. METHODS Women with a pre-pregnancy BMI ≥ 25 kg/m2 were given a fasted OGTT at 16–20 weeks (PRE), and again at 34–36 weeks of gestation (POST). The intervention consisted of a nutrition (ingestion of about 8350 kJ/day, 200 gm carbohydrate per day) and exercise (mild walking at 30% of peak VO2, 3 to 4 times per week) program (NELIP). RESULTS Women at risk for GDM at 16–20 weeks gestation (PRE) have a glucose area under the curve of 13.6±2.2 mmol/L/2hrs. None of the women at risk for GDM who participated in the NELIP developed GDM, with the POST program OGTT significantly lower than the GDM women (15.3±1.9 mmol/L/2hrs; vs 21.3±2.5p < 0.05). PRE and POST glycosylated hemoglobin (A1C) values remained within the non-diabetic range (A1C < 0.06) for those women at risk who participated in the NELIP (0.050±0.004 and 0.055±0.005, respectively). Women at risk for GDM who participated in NELIP had insulin areas under the curve (180.9±49.9ulU/ml/2hr) within the normal range at 34–36 weeks gestation. CONCLUSIONS NELIP may be an effective tool in helping to prevent GDM by regulating maternal blood glucose and insulin responses in women at risk for this disease.
