Abstract
Wirral Stroke Service MethodologyAll 769 stroke patients admitted to Wirral Hospital between 1/3/00 and 28/2/01 were assessed on the first working day after admission by a stroke coordinator nurse who recorded details for the stroke register and at their weekly progress review visit until discharge.The data on the 585 stroke patients who were discharged alive has been studied. Results168/585 (29%) of these had a length of hospital stay which exceeded 56 days.59 / 168 (35%) of those were discharged to residential care.The remaining 109 patients (8 of whom were aged <50) returned to their own home after prolonged rehabilitation.69/109 of these improved by at least 1 level on the Rankin scale during their stay.A further 23 improved by at least 1 level in the stroke register sub-scales for walking, transfers, stairs use, cognition, speech or swallowing ability. ConclusionProlonged rehabilitation or issues related to the organisation of future care appear to be the main reasons for a hospital stay >56 days after a stroke.Progress in rehabilitation often continues beyond day 56.The decision making process that residential care is needed is time consuming and often associated with non-clinical delays.
